It's very interesting that Toronto turned to Yamaguchi and Rafael Dolis over some of these guys with recent MLB production.

 

How Dolis and Shun perform will say a lot about Toronto's scouting/analytics/projection department.

 

You could also add Bass to that list , could you not?

Earlier today, LHP James Paxton underwent a microscopic lumbar discectomy with removal of a peridiscal cyst. The surgery was performed by Dr. Andrew Dossett in Dallas, Texas. His approximate timeline to return to Major League action is 3-4 months.

The Yankees opted for more conservative treatment before putting Paxton under the knife. Back surgery of this sort apparently doesn't have a 100% success rate, so conservative treatment is recommended first. After a flare-up of symptoms that couldn't be remedied with conservative means popped up then it was determined that surgery was the best course.

That doesn't make much sense. It's a herniated disc and a cyst - yoga or whatever is not going to fix it. That article says discectomy procedures have a 96% return to play rate in baseball, which seems like an excellent rate. Nothing has a 100% success rate. Return to play from a hangnail probably only has a success rate of like 99.9999%

If it were me I'd always opt for conservative treatment before going under the knife for a back injury. What if he falls into the 4% that doesn't make it back to competitive play? Then his career is finished, and he likely has a long term back issue to deal with during retirement. Might as well see if more conservative measures do the trick first, who gives a f*** if you miss a few months of play?

Well, the Yankees.

 

I'm essentially just saying that I bet the team wanted him to have the surgery earlier but Paxton did not want to so I agree with your personal sentiments from Paxton's perspective.

 

I'm really not sure what conservative treatment he could have even been trying. The effective treatments for disc issues basically just = opioids and injections. Physical therapy might alleviate some pain for normal people but for an athlete? Big doubt.

A specific exercise regimen and healing hands can often mitigate back issues ie. Estrada.

Maybe. Anyways, these things aren't necessarily in the team's control. The Yankees cannot force Paxton to go under the knife.

The feels like the kind of situation that will end up lingering all year and limit Paxton to like 5 September starts. Probably ends up accepting a QO from NY instead of trying to land 80M with a good 2020.

I'm not so sure I'd agree with you here. Estrada was able to pitch for several years with recurring back injuries. It's entirely possible the conservative treatments are what enabled him to pitch as long as he did, without them he very well may have been finished years ago. A quick glance at google shows that Estrada was dealing with back problems all the way back to at least spring training in 2016, he didn't end up retiring until many years later.
