They should be assigned as team errors. They manage individual and team turnovers in basketball and they should do the same in baseball.

I think "the play would have been made by an average fielder" is a reasonable rule. Errors are already a judgment call, so just expand to include team errors.

I think there's a chance, if he stays this efficient and doesn't get into any real trouble there's no need to use the pen. Though Chuckles DOES love changing pitchers at every possible opportunity (I'm very thankful for the 3 batters rule... )
