Should a rule be added re: the Renfroe thing? A ground-rule triple? Same rule as GRD but with one base added as punishment for losing the ball out of play. It would be about as costly a thing to happen besides a home run. What would the downsides be?

An alternative could be to allow all base runners to score, but put the batter at 2nd. That's probably more fair.

Ground rule triple sounds good. Automatic triple is already a thing (throwing your glove/equipment at a fair ball is an auto triple) so I don’t see why it can’t be extended to a defender misplaying a ball and booting it into the stands.
