Dude, it's the summer and it's the weekend and the team is playing like absolute s***. We can't be perfect.
We have 4 mods and even then we can't guarantee that 1 or 2 will be online all the time. And even when we are, a new account from a new IP address with a new name isn't always obviously a troll from the start and we can't ban almost every new account in sight. Any account that we can be very sure is Metallijay or Dogg gets banned now - those two are so far past the line that we have deemed them incorrigible and we all want them gone. They do get banned as soon as possible. When I'm online for hours at a time and ban one of them, I watch for the "newest member" who starts posting and am always suspicious it might be them obviously. I've lost count on how many we've shut down. It's endless. We are trying to send a strong message so that they'll stay away but these two just WON'T for whatever reason. Blame them, not the mods, OK?

We do our best. If a suspicious account pops us we watch it. We have a mod subforum where we discuss issues on an ongoing basis. If I'm 99% sure a new account is garbage I'm aggressive and will delete or ban it if a few posts show me that the person is clearly going to be a problem. In the rare circumstance that the person wasn't a known troll, we hope they'll come back with a new account and try to get along better.
If someone looks like trouble, report one of their posts. Admin, Chappy, Hurl and I will all get an e-mail immediately and know something is up (unless I'm at the cottage drinking beer and swimming like I was yesterday). We will get on it in a timely manner. I have banned countless accounts from tips by you guys. King has been very helpful in this regard. Reporting the problem posts/posters is FAR more effective than bickering with them and calling them out for being a troll and complaining publicly that they haven't been banned yet. In fact, you know what? The probably LOVE s*** like that, and s*** like this thread because they can see that they're annoying us which is all they're trying to do. It gives them unwarranted attention and feeds their notoriety. Report them. And let us clean up as best we can/see fit.
