I assume the insurance would be bought at the time the contract was signed, not on a year to year basis. -

I think most teams don't insure contracts anymore (or if they do it would only cover a career ending injury, not smaller stuff). -
I don't know, we probably would have had a billion mound visits by now without them.

I think with him placed on the 60 at the end of spring training, he has to go the full 60 days. -
