The problem is every possible gesture, topic, discussions can offend somebody even without intention. It's so easy to express words without any thought or meaning on social media and it's a completely open platform. A harmless gesture can offend 0.1% of the population and they become a vocal minority. Twitter has 330 million registered accounts. Even 99.9% of them don't care about a topic, offending 0.1% of those people is still 330,000 people. It's absurd. Even if just 5 people get offended, a voice that influences others & their tweet gets blown way out of proportion.
